Before You Move…

All Senior Communities (from Independent to Nursing Home) Did you know?will want a copy of your Health Care Proxy and Power of Attorney. It is smart to have these in order regardless of whether you are moving. Senior Life advises that you use an elderlaw attorney. They’ll also want a written 2 reflection of your financial situation. Senior Life recommends that you type up one listing and make copies for each application.

Thinking of moving to Assisted Style Living? They’ll need a 3122 form completed by your Primary Care Doctor. The “3122″ documents that you were seen by your doctor and s/he finds you appropriate for this level of care. The 3122 form is good for 30 days. They’ll also need you to have a PPD (tuberculosis) test placed and “read” by one of the doctor’s nurses. This needs to have been done in the 6 months prior to admission.

In short, if you have picked an apartment and are getting ready to move, call the doctor to get the 3122 and PPD done.

Thinking of moving to a Nursing Home? They’ll need a PRI completed. “PRI” is short for Patient Review Instrument. An assessment is performed, and a PRI document is completed by a certified professional. The PRI determines whether the individual meets the criteria for nursing home admission. (Nothing’s just easy is it?) A PRI is good for 90 days.
