1. Initial Consultation,
When something has changed, this is the place to begin:    $200.00
We will provide personal attention to you and your family as you face the changes that come with aging. We’ll be discussing various aspects of your life as it pertains to your current needs and the future. After we are sure we understand what is important to you: We will make recommendations, explain all your choices, and direct you to the best resources in town. You’ll walk away feeling that you have a better understanding of your situation, available options, and a Plan. You can leave it at that, or we can continue to help. See our SERVICE MENU for more considerations. We advise, assist and advocate. We respect diversity and the right to choose or refuse.

2. Expert R.N. Geriatric Assessment and Considerations:    $95/per hour
The experts at Senior Life will assess all body systems, memory function and review your medications. This Assessment can take place in your family home or within a health care community. We’ll spell out all your best options. We’ll connect the dots between you and your health care providers and follow up with a written report. The complete process usually entails a couple of hours with the Client themselves and an hour or two of follow up with providers, maybe family, and the written report. The complete process, (Up to 4 hours), is billed as a package.

3. Stand-by & Vacation Relief Club:    $25/Mon Membership + Quarterly Visits
Perfect for anyone who worries about the “what ifs” of aging: What if you go to the hospital? What if your doctor says you need a nursing home? What if something happens to you, who will care for your spouse? This Membership takes the worry out of these issues. Senior Life will keep a “crisis chart” ready. With one phone call from you or anyone on your health team, Senior Life will step in and act in your behalf in the way you desire the same day, (During the course of normal business hours). There is comfort in knowing Senior Life is ready to help. Imagine taking that well-deserved vacation, knowing your loved one is safe in our care. This Membership requires #1 and #2 above and a quarterly visit, (Billed at the hourly rate). This allows Senior Life to keep abreast of changes and offer preventative solutions as needed.

4. Chart Reviews, (& compilations of history):     Priced Individually
This Review is best for people who have had multiple care providers, (physicians or facilities) and /or hospitalizations. This is also a helpful resource for individuals who have relocated or plan to relocate. This is perfect for anyone who would like a specialist to provide an assessment, give professional considerations and to compile a complete history for future reference. We will compile a complete report for your use or to distribute to the physician team. We’ll discuss our findings at a Care Plan Meeting with you and or your Health Team. This is a valuable addition to quality care.
