Senior Life Packages and Prices

PACKAGES AND PRICES

LEVEL of CARE DETERMINATION and OPTION REVIEW:

1. ELDER IN-HOME ASSESSMENT (family member must be present- copy of medication list and insurance along with family questionnaire completion is required)
2. FAMILY CONSULTATION (separate appointment to share assessment results and level of care considerations)
COST: $ 180.00/hour
ESTIMATED TIME: 3 - 5 hours



APPLICATION TO ASSISTED LIVING, MEMORY CARE OR NURSING HOME:

1. REFERRAL TO ELDER LAW ATTORNEY - if required / helpful.
2. PRIMARY DOCTOR – we collect necessary documents.
3. APPLICATION – POA will complete this form.
4. COORDINATION and TRACKING – we monitor and encourage each step of the process (with family, elder and senior community).
5. SENIOR COMMUNITY and FAMILY COMMUNICATIONS - for optimum results.
COST: $ 180.00/hour
ESTIMATED TIME: 3 - 6 hours

(This is dependent on how much of this we do for you and how much follow up is necessary.)

NOTE: If the elder is in the hospital, the social worker there will assist you.


STANDBY PROGRAM / MEMBERSHIP $55/month and EMERGENCY FUND DEPOSIT

$55/ month Membership Fee

•   Active Status – we will move a non-urgent active client schedule to accommodate your needs in a crisis or urgent situation.
•   Standby Members have extended service availability at the usual day rate. We will work up to 7PM and will work Saturday morning 9-11 for crisis/ urgent situations. (This is not available in any other level of service.)
•   First return call.

Senior Life LLC hourly rates are charged for the following:

•   We make contact every 6 months minimally to update our records. Family may update us on medications/ diagnosis/ hospital visits or we will call other caregivers.
            Estimated time: 30-45 minutes. (more only if there are complications.)

•   Family makes contact with updates as they occur.
            Estimates: 10-minute one-time call, no charge.
            All other calls are timed precisely and charged accordingly.

•   One time or brief requests for services.


STANDBY Stops:

•   When we receive your request to cancel.
•   If the client has ongoing services, Standby payments stop and your file becomes active.
•   Standby restarts only on your approval.


EMERGENCY FUND DEPOSIT

Senior Life requests $1,000.00 on retainer for all Standby Applicants and for all elders who pay our invoices directly. The purpose of this retainer is to cover our expenses in a timely manner when there is an urgent need. We will jump in and work hard for you. During the time of a crisis, you don’t want to worry about bills, but as a small business, we appreciate getting paid on time. Any unused portion will be returned in full at your request or at the termination of services.
